Jason Lewis, M.D.
Dr. Lewis was born and raised in Zionsville, Indiana. After receiving his bachelor's degree in microbiology from Purdue University, he received his doctor of medicine degree from the Indiana University Medical School. Dr. Lewis completed his residency training in family medicine at the St. Vincent Family Medicine Program in Indianapolis.
As a family medicine physician, Dr. Lewis enjoys seeing patient of all ages and has a special interest in rural and international medicine.
Dr. Lewis enjoys playing golf, projects around the home, and spending time with his wife, Bethany, and daughter, Hadley. Dr. Lewis and his wife are expecting their second child at the end of the year.
Laura Clarke Cridge, R.N., MSN, FNP, NP-C
Laura received her bachelor’s degree in nursing from Ball State University in 2002. She worked in the Intensive Care Unit at Ball Memorial Hospital and then joined Community Health Network at Community Hospital Anderson in the Same-Day Surgery and Post Anesthesia Care Unit. She worked there while she obtained her master's degree in nursing to become a family nurse practitioner, graduating in July of 2010. Laura is nationally certified as a family nurse practitioner by the American Academy of Nurse Practitioners.
Laura’s focus as a nurse practitioner is a holistic approach to the care of each patient that encompasses the whole person, with an emphasis on health promotion and disease prevention. She practices in collaboration with the physician to assess, diagnose, treat and manage the patient’s health problems and needs.
Laura grew up in New Castle, Indiana, where she resides now with her husband and their five children. She enjoys reading and spending time with her family, especially when the whole family sits down to a meal together. Because of her big family, Laura understands the importance of treating not only a single patient, but the whole family.
